Here's a map of the Kingdom Hearts universe, as I see it when I write my stories. Basically, I took the layout and words of the second game, added the worlds form the first where they seemed to fit most practically (I assumed the universe sort of condensed when worlds started disappearing), and then added some additional worlds used in my stories.

Disclaimer: I do not own any rights to Kingdom Hearts or any of the Disney & Square Enix locations seen here.

Edit (29-8-08): I added a world and made the world's titles a little easier to read.

The additional worlds are:
The Rose Kingdom = Sleeping Beauty
The Enchanted Woods = Snow White
The Land of Dreams = Cinderella
Nottingham = Robin Hood
Notre Dame = The Hunchback of Notre Dame
The Jungle = The Jungle Book
The Empire of the Sun = The Emperor's New Groove
Gaia = Various Final Fantasy cities (obviously not the ones seen in Radiant Garden -see map linked above)
Andalasia= The animated world from Enchanted
Narnia, Atlantis, and Treasure Planet are the worlds from the films by the same titles.
